Provider information and data policy
Version v1.0. Last updated September 2026. This is the notice you accept when you submit the provider credentialing form. Processed under the Protection of Personal Information Act, 2013 (POPIA).
Who collects your information
Network One Health (Pty) Ltd manages Dr H P Manyonga & Associates Inc. (BHF practice number 0977357) and associated practices. Credentialing is carried out by named administrative staff and the practice manager, under the practice's clinical governance.
What we collect
When you apply to join as a provider, we collect:
- your name, email address and phone number;
- your professional designation and current occupation;
- your professional council registration number (HPCSA, SANC or the council you register with);
- your indemnity provider and policy number;
- where you practise;
- a copy of your identity document and your council registration card;
- your signature and the date you submitted the form;
- contact details for referees, where you give them to us.
Why we collect it
To confirm that you are registered to practise, that your registration is current and unrestricted, and that you carry professional indemnity cover. We also use it to meet the requirements of the funders and hospitals we work with, and to decide whether and how you can work with us. We may also add your email address to our provider mailing list, to send you updates about working with us. You can unsubscribe from that list at any time, and doing so does not affect your application.
How we verify it
We check your registration against the official register of your professional council. We confirm your indemnity cover is current. Where a practice number applies, we validate it. We contact the referees you give us, and we may carry out a competence assessment before you see patients. Verification is done by the practice manager or a suitably qualified clinician, not by administrative staff alone.
Who sees it
Named administrative staff who file it, the practice manager who verifies it, and the responsible director. We disclose it outside the practice only where a hospital, funder or regulator requires it for your accreditation or for a lawful purpose, or where the law obliges us. We do not sell it or share it for any other purpose.
Where it is kept
In the practice's controlled storage, with access limited to the people named above. Your documents are not kept in personal mailboxes or on personal devices. We keep your credentialing records for as long as you work with us, and afterwards for as long as the law, a funder or our insurers require.
What this notice is not
Accepting this notice is not a contract to provide services. The commercial terms, including fees, obligations, notice periods and dispute resolution, are set out in a Service Level Agreement. We send that agreement to you separately, you are free to review and negotiate it, and it takes effect only when both parties sign it. Submitting the credentialing form does not commit you to those terms, and it does not commit us to appoint you.
Your rights
You may ask to see the information we hold about you, ask us to correct it, or ask us to delete it where we are not required to keep it. You may withdraw your consent, though we may then be unable to continue credentialing you. An access request may be subject to a legally allowable fee to cover costs. You may also complain to the Information Regulator of South Africa.
